Reporting a Significant Injury on Campus

Know when you must report injuries occurring on campus.

Specific significant injuries are required to be reported immediately to the Illinois Occupational Safety and Health Administration (IL OSHA).

  • Was the injury fatal?
  • Was the worker admitted to the hospital?
  • Was there an amputation?
  • Did someone lose an eye?

If the answer to any of these questions is YES, immediately call the Division of Safety and Compliance at
217-255-2106 and provide:

  • Building name and address where the incident took place
  • Date and time of the incident
  • Date and time of notification to supervisor/department official
  • Number of fatalities or hospitalized employees
  • Name and contact number of the person making the report
  • Brief description of the incident

A flyer outlining the requirements should be printed and placed in locations where other important employee information is also displayed. For questions, contact oshs@illinois.edu, 217-265-9828.
